Payment for Services
Please plan to take care of co-payments, co-insurance and any outstanding balances at the time of service. Co-payments or deductibles must be paid at the time of visit. We accept debit cards, Visa and Mastercard.
If we are not a participating provider with your insurance plan, or your provider does not cover services provided, you will be responsible for any charges incurred. We will provide you a receipt for your insurance company.
We realize that emergencies do arise that may affect timely payment of your account. If such extreme cases do occur, please contact a patient account representative at (435) 723-0329.
Any check returned from the bank will result in an additional $20 charge.
Always notify our office of any change in name, address, phone or insurance information.
For your first visit, please bring your insurance card and arrive 30 minutes early to complete the necessary patient information forms.
We accept Medicare as well as most insurance plans. Prior to your appointment, please verify with your insurance company that our physicians are participating providers and ask if a referral is necessary.
If you are being referred, please be sure to bring the referral form (if required by your insurance company) to your appointment.
